New EU project aims to prevent hospital admissions among older citizens

Professor Annelise Norlyk from the Department of Public Health has, together with international colleagues, received a grant of 10 million DKK from the EU and Innovation Fund Denmark for a new project aimed at reducing avoidable hospital admissions among older citizens.

Many older citizens receiving home care or living in residential care facilities are admitted to hospital with conditions that could potentially have been prevented. An unnecessary hospital admission can be a distressing and disruptive experience that negatively affects quality of life, and it also places a significant financial burden on the healthcare system.

In the CARE-CDS project, researchers will investigate what happens in the period leading up to a hospital admission and how care practices can be strengthened to reduce avoidable admissions.

The project will involve analysing data from care and treatment documentation, mapping workflows and collaboration between professional groups, and incorporating experiences from citizens, relatives, and healthcare professionals.

“We assume that many hospital admissions among older citizens can be prevented through better coordination, early detection, and increased use of evidence-based knowledge in care,” says Professor Annelise Norlyk.

The aim of CARE-CDS is therefore to develop and test solutions, including digital decision-support tools, to support better decision-making and improve quality, continuity, and safety in elderly care.

The project is carried out through an international collaboration between research institutions in Denmark, Norway, Austria, and Portugal, as well as the company VAR Healthcare and municipal practice partners.
